The Process of Egg Donor

Egg donation is the process of a young woman giving her eggs to an infertile couple that desires to have children. This donation is part of third party reproduction, in which a pregnancy is the goal by in vitro fertilization.

An egg donor may be motivated by a number of reasons to donate her eggs. Some egg donors may feel that by donating they provide a benefit for another couple. Others may be helping an infertile couple that they know or decide to donate from another infertility experience. Some donors are attracted to the monetary compensation.

Women that are infertile may need an egg donor for any number of reasons. Infertile couples may need an egg donor because the female partner cannot have biological children due to her eggs being unfertile. Another problem which may cause couples to seek out an egg donor would be a genetic disorder that the couple does not wish to pass on to their child. Or the female partner may simply be past her child-bearing years and is unable to conceive because of menopause or age-related factors.

Couples can recruit a donor that they know or use an anonymous donor. Couples can also choose an egg donor through a database with pictures, personality information, and a family medical history about the donor. All egg donors are extensively screened for a healthy medical history, and even recruited for potential characteristics such as a high IQ, college education, musical or athletic ability, and beauty. Some donors can gain higher monetary compensation for their services due to their exceptional abilities, some clinics even givie exceptional donors the ability to choose their price (up to $15,000) for their time and effort (particularly for ethnic donors that are in high demand). Higher monetary compensation is very common in states such as California, Arizona, and east coast states where Ivy League schools are present.

Once the egg donor is accepted into the program (after doctor tests and a psychological exam), she undergoes the in vitro fertilization stimulation therapy. The IVF stimulation involves daily injections of fertility drugs to boost the production of the donor’s eggs. After several weeks of injections and doctor appointments, the final step for the donor is the egg retrieval procedure. The procedure involves retrieval through a needle inserted through the vaginal while the donor is sedated. After egg retrieval, the ova are fertilized by the sperm of the male partner in the laboratory, and after several days, the resulting embryo(s) is placed in the uterus of the female recipient. During this time, the donor and the recipient have been prepared for the embryo transfer. The female recipient will hopefully conceive and carry that baby to term. The recipients will raise the child as their own.

Since the inception of IVF treatments, egg donor recipients often have a 50% chance of success. Personal factors may affect the treatment, but success has been known by women of all ages and conditions. The oldest woman to give birth by egg donation was 66 years old. With advanced technology, fertility services can help many infertile couples conceive through an egg donor.

IVF is the scientific approach to getting pregnant. Originally termed "test-tube" babies by the Press, embryos are fertilized in the lab, removed from a human body. One attempt at pregnancy through IVF is termed a cycle, and can be divided into 5 general phases.

1. Preparation

You're body is manipulated with drugs. Doctors prescribe a medication such as Lupron to shut down your ovaries for two weeks.

2. Stimulation

Following the Lupron series you will receive a set of up to 14 shots of another medication, such as pergonal, to hyperstimulate egg production. At the conclusion of these shots you will be given a final medication to boost maturity of your eggs.

3. Harvesting

Once the eggs have reached maturity you are heavily sedated and between 5-15 eggs are suctioned from your ovaries via ultrasound guided vaginal retrieval.

4. Fertilization

Egg and sperm meet each other for the first time in the lab. Approximately 100,000 motile sperm are introduced to each egg. Fertilization is documented and the growing embryos are carefully observed in vitro for up to 6 days. The growing trend is to observe growth longer, past the 6-8 cell stage, and blastocyst or advanced stage embryo transfer is not uncommon. 5. Embryo Transfer

Mom comes back in the picture as 3 or 4 growing embryos are transferred back into your uterus in a procedure that resembles a PAP smear. With any luck a new baby begins growing. On average it takes 3 IVF cycles to establish a pregnancy.

Latar Belakang Munculnya Inseminasi Buatan (Bayi Tabung)

Pelayanan terhadap bayi tabung dalam dunia kedokteran dikenal dengan istilah fertilisasi-in-vitro yang memiliki pengertian sebagai berikut : Fertilisasi-in-vitro adalah pembuahan sel telur oleh sel sperma di dalam tabung petri yang dilakukan oleh petugas medis. Inseminasi buatan pada manusia sebagai suatu teknologi reproduksi berupa teknik menempatkan sperma di dalam vagina wanita, pertama kali berhasil dipraktekkan pada tahun 1970. Awal berkembangnya inseminasi buatan bermula dari ditemukannya teknik pengawetan sperma. Sperma bisa bertahan hidup lama bila dibungkus dalam gliserol yang dibenamkan dalam cairan nitrogen pada temperatur -321 derajat Fahrenheit.

Pada mulanya program pelayanan ini bertujuan untuk menolong pasangan suami istri yang tidak mungkin memiliki keturunan secara alamiah disebabkan tuba falopii istrinya mengalami kerusakan yang permanen. Namun kemudian mulai ada perkembangan dimana kemudian program ini diterapkan pula pada pasutri yang memiliki penyakit atau kelainan lainnya yang menyebabkan tidak dimungkinkan untuk memperoleh keturunan.

Otto Soemarwoto dalam bukunya “Indonesia Dalam Kancah Isu Lingkungan Global”, dengan tambahan dan keterangan dari Drs. Muhammad Djumhana, S.H., menyatakan bahwa bayi tabung pada satu pihak merupakan hikmah. Ia dapat membantu pasangan suami istri yang subur tetapi karena suatu gangguan pada organ reproduksi, mereka tidak dapat mempunyai anak. Dalam kasus ini, sel telur istri dan sperma suami dipertemukan di luar tubuh dan zigot yang terjadi ditanam dalam kandungan istri. Dalam hal ini kiranya tidak ada pendapat pro dan kontra terhadap bayi yang lahir karena merupakan keturunan genetik suami dan istri.

Akan tetapi seiring perkembangannya, mulai timbul persoalan dimana semula program ini dapat diterima oleh semua pihak karena tujuannya yang “mulia” menjadi pertentangan. Banyak pihak yang kontra dan pihak yang pro. Pihak yang pro dengan program ini sebagian besar berasal dari dunia kedokteran dan mereka yang kontra berasal dari kalangan alim ulama. Tulisan ini tidak akan membahas mengenai pro kontra yang ada tetapi akan membahas mengenai aspek hukum perdata yang menekankan pada status hukum dari si anak dan segala akibat yang mengikutinya.

Proses Inseminasi Buatan (Bayi Tabung)

Dalam melakukan fertilisasi-in-virto transfer embrio dilakukan dalam tujuh tingkatan dasar yang dilakukan oleh petugas medis, yaitu :
1. Istri diberi obat pemicu ovulasi yang berfungsi untuk merangsang indung telur mengeluarkan sel telur yang diberikan setiap hari sejak permulaan haid dan baru dihentikan setelah sel-sel telurnya matang.
2. Pematangan sel-sel telur sipantau setiap hari melalui pemeriksaan darah Istri dan pemeriksaan ultrasonografi.
3. Pengambilan sel telur dilakukan dengan penusukan jarum (pungsi) melalui vagina dengan tuntunan ultrasonografi.
4. Setelah dikeluarkan beberapa sel telur, kemudian sel telur tersebut dibuahi dengan sel sperma suaminya yang telah diproses sebelumnya dan dipilih yang terbaik.
5. Sel telur dan sperma yang sudah dipertemukan di dalam tabung petri kemudian dibiakkan di dalam lemari pengeram. Pemantauan dilakukan 18-20 jam kemudian dan keesokan harinya diharapkan sudah terjadi pembuahan sel
6. Embrio yang berada dalam tingkat pembelahan sel ini. Kemudian diimplantasikan ke dalam rahim istri. Pada periode ini tinggal menunggu terjadinya kehamilan.
7. Jika dalam waktu 14 hari setelah embrio diimplantasikan tidak terjadi menstruasi, dilakukan pemeriksaan air kemih untuk kehamilan, dan seminggu kemudian dipastikan dengan pemeriksaan ultrasonografi.

Permasalahan Hukum Perdata yang Timbul Dalam Inseminasi Buatan (Bayi Tabung)

Inseminasi buatan menjadi permasalahan hukum dan etis moral bila sperma/sel telur datang dari pasangan keluarga yang sah dalam hubungan pernikahan. Hal ini pun dapat menjadi masalah bila yang menjadi bahan pembuahan tersebut diambil dari orang yang telah meninggal dunia. Permasalahan yang timbul antara lain adalah :
1. Bagaimanakah status keperdataan dari bayi yang dilahirkan melalui proses inseminasi buatan?
2. Bagaimanakah hubungan perdata bayi tersebut dengan orang tua biologisnya? Apakah ia mempunyai hak mewaris?
3. Bagaimanakah hubungan perdata bayi tersebut dengan surogate mother-nya (dalam kasus terjadi penyewaan rahim) dan orang tua biologisnya? Darimanakah ia memiliki hak mewaris?

Tinjauan dari Segi Hukum Perdata Terhadap Inseminasi Buatan (Bayi Tabung)

Jika benihnya berasal dari Suami Istri
· Jika benihnya berasal dari Suami Istri, dilakukan proses fertilisasi-in-vitro transfer embrio dan diimplantasikan ke dalam rahim Istri maka anak tersebut baik secara biologis ataupun yuridis mempunyai satus sebagai anak sah (keturunan genetik) dari pasangan tersebut. Akibatnya memiliki hubungan mewaris dan hubungan keperdataan lainnya.
· Jika ketika embrio diimplantasikan ke dalam rahim ibunya di saat ibunya telah bercerai dari suaminya maka jika anak itu lahir sebelum 300 hari perceraian mempunyai status sebagai anak sah dari pasangan tersebut. Namun jika dilahirkan setelah masa 300 hari, maka anak itu bukan anak sah bekas suami ibunya dan tidak memiliki hubungan keperdataan apapun dengan bekas suami ibunya. Dasar hukum ps. 255 KUHPer.
· Jika embrio diimplantasikan ke dalam rahim wanita lain yang bersuami, maka secara yuridis status anak itu adalah anak sah dari pasangan penghamil, bukan pasangan yang mempunyai benih. Dasar hukum ps. 42 UU No. 1/1974 dan ps. 250 KUHPer. Dalam hal ini Suami dari Istri penghamil dapat menyangkal anak tersebut sebagai anak sah-nya melalui tes golongan darah atau dengan jalan tes DNA. (Biasanya dilakukan perjanjian antara kedua pasangan tersebut dan perjanjian semacam itu dinilai sah secara perdata barat, sesuai dengan ps. 1320 dan 1338 KUHPer.)

Jika salah satu benihnya berasal dari donor

· Jika Suami mandul dan Istrinya subur, maka dapat dilakukan fertilisasi-in-vitro transfer embrio dengan persetujuan pasangan tersebut. Sel telur Istri akan dibuahi dengan Sperma dari donor di dalam tabung petri dan setelah terjadi pembuahan diimplantasikan ke dalam rahim Istri. Anak yang dilahirkan memiliki status anak sah dan memiliki hubungan mewaris dan hubungan keperdataan lainnya sepanjang si Suami tidak menyangkalnya dengan melakukan tes golongan darah atau tes DNA. Dasar hukum ps. 250 KUHPer.
· Jika embrio diimplantasikan ke dalam rahim wanita lain yang bersuami maka anak yang dilahirkan merupakan anak sah dari pasangan penghamil tersebut. Dasar hukum ps. 42 UU No. 1/1974 dan ps. 250 KUHPer.

Jika semua benihnya dari donor

· Jika sel sperma maupun sel telurnya berasal dari orang yang tidak terikat pada perkawinan, tapi embrio diimplantasikan ke dalam rahim seorang wanita yang terikat dalam perkawinan maka anak yang lahir mempunyai status anak sah dari pasangan Suami Istri tersebut karena dilahirkan oleh seorang perempuan yang terikat dalam perkawinan yang sah.
· Jika diimplantasikan ke dalam rahim seorang gadis maka anak tersebut memiliki status sebagai anak luar kawin karena gadis tersebut tidak terikat perkawinan secara sah dan pada hakekatnya anak tersebut bukan pula anaknya secara biologis kecuali sel telur berasal darinya. Jika sel telur berasal darinya maka anak tersebut sah secara yuridis dan biologis sebagai anaknya.

Dari tinjauan yuridis menurut hukum perdata barat di Indonesia terhadap kemungkinan yang terjadi dalam program fertilisasi-in-vitro transfer embrio ditemukan beberapa kaidah hukum yang sudah tidak relevan dan tidak dapat meng-cover kebutuhan yang ada serta sudah tidak sesuai lagi dengan perkembangan yang ada khususnya mengenai status sahnya anak yang lahir dan pemusnahan kelebihan embrio yang diimplantasikan ke dalam rahim ibunya. Secara khusus, permasalahan mengenai inseminasi buatan dengan bahan inseminasi berasal dari orang yang sudah meninggal dunia, hingga saat ini belum ada penyelesaiannya di Indonesia. Perlu segera dibentuk peraturan perundang-undangan yang secara khusus mengatur penerapan teknologi fertilisasi-in-vitro transfer embrio ini pada manusia mengenai hal-hal apakah yang dapat dibenarkan dan hal-hal apakah yang dilarang.

Kasus Inseminasi Buatan di Amerika Serikat
Mary Beth Whitehead sebagai ibu pengganti (surrogate mother) yang berprofesi sebagai pekerja kehamilan dari pasangan William dan Elizabeth Stern pada akhir tugasnya memutuskan untuk mempertahankan anak yang dilahirkannya itu. Timbul sengketa diantara mereka yang kemudian oleh Pengadilan New Jersey, ditetapkan bahwa anak itu diserahkan dalam perlindungan ayah biologisnya, sementara Mrs. Mary Beth Whitehead (ibu pengganti) diberi hak untuk mengunjungi anak tersebut.

Negara Lain
Negara yang memberlakukan hukum islam sebagai hukum negaranya, tidak diperbolehkan dilakukannya inseminasi buatan dengan donor dan dan sewa rahim. Negara Swiss melarang pula dilakukannya inseminasi buatan dengan donor. Sedangkan Lybia dalam perubahan hukum pidananya tanggal 7 Desember 1972 melarang semua bentuk inseminasi buatan. Larangan terhadap inseminasi buatan dengan sperma suami didasarkan pada premis bahwa hal itu sama dengan usaha untuk mengubah rancangan ciptaan Tuhan.

Some Causes of Infertility in Woman

There are many factors that will relate how a woman develops infertility. While it is prevalent among Americans, no data can truly present the actual intensity or prevalence of this condition.

Infertility is definitely not a physical disease. Unlike simpler conditions like flu or the more complex ones such as those of cancer, symptoms of infertility are not focused on the obvious signs.

In fact, a woman need not undergo a series of comprehensive tests and examinations before she can truly be diagnosed of infertility. The same goes with men only differing in one thing, male infertility is much more difficult to be spotted unless obvious presentations of erectile dysfunction are seen.

Pelvic Inflammatory Disease or PID

This is presumed to be the most common cause of infertility. This arises from internal infections that are caused by bacteria penetrating into the internal reproductive organs of a female. The typical organs affected are those surrounding the pelvic area but when aggravated, infections may also radiate into the neighboring intestines. Infertility associated with PID is definite if the portion affected is the fallopian tube, a condition that is medically termed as salpingitis.

Endometriosis

According to data gathered from medical literature, nearly 30% of all infertility cases in women is covered by this condition. This is characterized with the presence of the endometrial tissue in parts other than the uterus. This tissue is the one women discharge during menstrual cycle.

Having this condition however does not actually suggest the likelihood of being unable to conceive. But it may largely contribute to the development of the disease

Polycystic Ovarian Syndrome

This is the condition characterized by the over-production of androgens in the female's system. This occurrence will drive the lowering in the release of other hormones such as Follicle Stimulating Hormone and Luteinizing Hormone which will eventually caused the stoppage of mature egg production.

Early Menopause or Premature Ovarian Failure

This is the premature depletion of follicles in women during ages prior to her 40th years. This is characterized by long periods of irregular menstrual flow. This condition is very much comparable with true menopause since both impede a woman to produce eggs.

Idiopathic Hypogonadotropic Hypogonadism

This is rarely the case among infertile women. This is identified when there is 'no' production of LH and FSH. Thus, the impossibility of developing egg cells. There are actually no physical symptoms that will help conclude the presence of this condition. Most cases of Idiopathic Hypogonadotropic Hypogonadism fall under unknown infertility cases.

Perokok punya Bayi Tabung Sehat?

Rokok berkaitan erat dengan kemampuan reproduksi. Salah satunya, adalah kemampuan seorang pria untuk menghasilkan sperma yang berkualitas. Berbagai penelitian membuktikan bahwa rokok bisa menurunkan kualitas sperma. Ini sangat masuk akal mengingat dalam sebatang rokok terdapat sekitar 4.000 partikel kimia yang berbahaya bagi tubuh alias beracun.
Secara sederhana dapat dijelaskan bahwa racun dalam rokok ini bisa masuk ke testis, sehingga mengganggu perkembangan sperma. Partikel tersebut masuk ke darah dan semen. "Kondisi pada sperma akibat rokok itu bisa dilihat dari jumlah spermanya yang berkurang, gerakannya menjadi lambat, dan bentuknya menjadi jelek," jelas embriologis dari Klinik Infertilitas Permata Hati RS Dr Sardjito, dr Ita Fauzia Hanoum, MCE.
Ada penelitian yang bisa membuktikan kalaupun bentuk sperma tidak bermasalah, kemudian gerakannya tidak terlalu berpengaruh, jumlahnya juga tidak terlalu turun, tetapi DNA-nya rusak. Jadi, kata dia, sekarang yang menjadi perhatian para perokok, terutama perokok berat, adalah kemungkinan tidak punya anak.
Penelitian yang mengemukakan bahwa rokok dapat mempengaruhi kualitas sel DNA sperma pria banyak membuat papa pria tersebut memilih jalur bayi tabung dalam hal memperoleh keturunan, tapi lewat proses bayi tabung-pun, DNA sel sperma yang sudah rusak akan tetap mempengaruhi proses kelahiran bayi tabung.
Kerusakan DNA itu bisa mempengaruhi banyak hal, bisa yang minor sampai ke mayor.
"Angka keguguran menjadi tinggi. Kalau ayah dan ibunya merokok, kontribusinya menjadi dua, tetapi kalau ibunya tidak merokok, angka kegugurannya karena DNA ayahnya rusak," tandasnya. Parahnya lagi, kalau si ibu tidak mengalami keguguran dan anak lahir hidup, anaknya mungkin ada kecacatan tertentu. "Apalagi bila si isteri usainya sudah lanjut, suami merokok, angka kecacatan anaknya akan semakin tinggi," jelasnya.
Di Klinik Permata Hati belum dilakukan penelitian tentang hubungan laki-laki yang ikut program bayi tabung dengan perilaku merokok, tetapi mereka selalu ditanya apakah mereka merokok atau tidak. Disarankan pula kepada para suami perokok yang ikut program bayi tabung agar tidak merokok. ''Memang ada yang mempertimbangkan hal itu, tetapi sebagian besar menganggap tidak ada pengaruhnya dan tidak peduli. Mereka tetap merokok. Padahal pendidikan mereka menengah ke atas,'' ungkap Ita. Dari hasil penelitian juga didapatkan bahwa suami perokok kemampuan untuk menghamili isterinya lebih lambat daripada suami yang tidak merokok. Kalaupun sang istri bisa hamil, masalah lain akan menanti di depan mata. Maka dari itu cara bayi tabung pun tetap tidak akan memberikan hasil yang baik jika anda tetap merokok.

Bayi Tabung Lebih Pintar?

Penelitian pertama terhadap anak-anak usia delapan tahun dari hasil pembuahan melalui metode intracytoplasmic sperm injection (ICSI) atau bayi tabung menunjukkan bahwa mereka rata-rata memiliki tingkat intelegensi yang lebih baik daripada anak-anak hasil reproduksi normal. Hal tersebut menolak anggapan bahwa teknik tersebut tidak seaman metode in vitro vertilization (IVF) standar yang biasa dipakai untuk menghasilkan bayi tabung.

ICSI dilakukan dengan menyuntikkan sperma secara langsung ke dalam sel telur, berbeda dengan IVF standar yang hanya meletakkan sperma sedekat mungkin dengan sel telur, tanpa disuntikkan, agar dapat melakukan pembuahan secara alami.

Beberapa penelitian pendahuluan yang dilakukan sejak 1998 melaporkan bahwa anak-anak hasil bayi tabung/ICSI usia satu tahun terlambat berkembang dibandingkan anak-anak yang normal. Sehingga keamanan teknik tersebut sempat diragukan. Tapi, penelitian yang lebih lama terhadap anak usia lima tahun, tidak ditemukan perbedaan tingkat perkembangan yang signifikan.

Baru-baru ini, tim yang dipimpin Lize Leunens dari Free University of Brussels (VUB) di Belgia membandingkan antara tingkat intelegensi dan kemampuan motorik terhadap 151 anak hasil bayi tabung usia delapan tahun dengan 153 anak hasil pembuahan normal.

Hasilnya, tidak ada perbedaan dalam kemampuan motorik dan anak-anak ICSI memiliki nilai tes intelegensi yang lebih tinggi daripada yang normal. Leunens memaparkan hasil penelitiannya dalam pertemuan tahunan Perkumpulan Reproduksi Manusia dan Embriologi Eropa di Kopenhagen, Denmark, Selasa (21/6).

"Kami sangat gembira karena dalam jangka panjang anak-anak hasil bayi tabung tersebut tidak menderita kemunduran dalam perkembangannya," katanya.

Dalam penelitian tersebut, tidak ada perbedaan level pendidikan dari ibunya, yang diketahui mempengaruhi tingkat intelegensi seorang anak. Oleh karena itu Leunens berpendapat bahwa alasan yang dapat menerangkan adalah motivasi yang lebih besar dari ibu yang mengandung bayi ICSI. "Ibu yang mengandung bayi ICSI ini mungkin mendedikasikan dirinya secara khusus sebagai orang tua," katanya.

Selain itu, penjelasan yang masuk akal juga disampaikan menanggapi kemunduran tingkat perkembangan pada bayi ICSI yang berusia sangat muda. Beberapa penelitian menunjukkan bahwa ibu bayi ICSI lebih suka membesarkan anaknya di rumah daripada mengirimkan ke playgroup atau berinteraksi dengan orang lain, kondisi yang mungkin menyebabkan kemunduran dalam perkembangan sosial.

Tapi, penelitian ini bukanlah jawaban terakhir. Penelitian lain menunjukkan bahwa penolakan banyak orang tua untuk mengijinkan anaknya diteliti, mungkin agak menurunkan kepercayaan hasil penelitian Leunens. Faktanya, sepertiga orangtua anak-anak ICSI menolak berpartisipasi.

Tanpa mengesampingkan kemungkinan-kemungkinan yang lain, Leunens menyatakan bahwa hasil penelitian tidak berbeda dengan kondisi yang dipaparkan orang tua melalui wawancara telepon. Ia juga menekankan bahwa penelitiannnya tidak melihat masalah kesehatan yang lain.(NewScientist.com/Wah)

Tehnik Bayi Tabung: Bedah Laparoskopik

Dalam proses bayi tabung secara ICSI, GIFT atau ZIFT seringkali ada operasi bedah laparoskopik (laparoscopic surgery). Ini adalah sedikit pembahasan mengenai laparoscopic surgery tersebut.

Operasi bedah laparoskopik merupakan teknik bedah yang dilakukan dengan cara membuat lubang kecil di dinding perut dan mengangkat kandung empedu dengan instrumen khusus menggunakan sistem endokamera melalui layar monitor.
Operasi ini digunakan dalam prosedur bayi tabung untuk memasukkan sel telur yang sudah dibuahi oleh sel sperma dan berkembang menjadi zigot ke dalam tuba fallopi si pasien wanita untuk kemudian agar dapat tumbuh secara alamiah menjadi bayi.

Efek bedah laparoskopik merupakan kebalikan dari efek bedah konvensional yang seringkali menimbulkan rasa nyeri pasca operasi, munculnya bekas pembedahan, masa pulih yang lambat, dan masa rawat yang panjang. Imagine that you can choose what your brother or sister would look like. Imagine if you could choose your child's eye color, hair color, IQ level, and even his interests. Imagine a world where everyone is perfectly healthy and wise IQ-wise.

This seems like a scene from a science fiction movie? Well, it might be possible in the near future to the promotion of preimplantation genetic diagnosis or PGD. PGD is a procedure in which the embryo is genetically tested to be free of the disease and then implanted in the mother using invitro fertilization (IVF).

Parents can now choose to have the ultimate child who excelled in both studies and sports. We all got a glimpse of IPRs in the 1997 film Gattaca where genetic engineering and IVF enabled engineering children, including factors such as gender, intelligence, life and even eliminate several genetic and hereditary diseases.

However, the DPI does not exist only in science fiction films. In fact, the DPI continues to develop in an exciting technology with the incredible potential to increase women's opportunities and not just a healthy pregnancy, but a healthy infant as well.

The DPI has been made famous by Dr. Jeffrey Nisker, the University of Ontario. He launched the IPR, and was the first person in Canada to offer such a procedure revolutionary engineering manually embryos. Since then, he has stopped its program and closed down due to overwhelming requests from across Canada to access PGD, not to avoid serious genetic conditions for sex selection, but above all to avoid having a daughter.

It is against its concern and belief that all children should be cherished, regardless of their sex. Among the cases in which the DPI was made, when a couple needed the procedure of having a baby with the right genetic makeup to save the life of their son. Leanne and Stephen needed the procedure to save their first-born, BJ, who suffers from a rare genetic disease, Hyper IGM. BJ is missing a vital part of his immune system and survives only through blood transfusions regularly.

The child needs to be free of disease and of the same sex as BJ, only then they can be assured that the tissue can be transplanted to the newborn BJ. The procedure is long and tedious with Leanne and Stephen go through a roller coaster of emotions that the fertilized eggs are extracted and then sent for testing.

After a few rounds missing, Leanne and Stephen made a hit and the embryo was transplanted into Leanne. Soon, their baby will arrive, bringing aid and hope to his older brother and parents! The DPI has nevertheless raised some ethical concerns of society, especially with regard to its potential and who should decide how to use this revolutionary new technology.

The procedure allows parents to determine the sex of the embryo, and thus can be used to select the sex of an embryo, preferably to another within the framework of "family balancing".

The company also raised the issue of doctors and scientific "playing God", which interferes with the natural shape of the design can have a major impact on our planet in the future.

Many women find IVF be a source of stress and anxiety-ridden experience. These factors often result in undermining the success of IVF. However, on a positive note, a 2002 German study reported that 34 out of 80 women who have been the subject of an IVF and were also receiving acupuncture got pregnant-a success rate of 42.5 for cent. The condition of the fallopian tubes, the length they were left and thus what the doctor has left to reconnect, and how well they are reconnected all play a part in how soon you can get pregnant and what your chances of getting pregnant are.

How long since the tubal ligation surgery was carried out? - Unfortunately the longer a woman waits in order to get her tubal ligation procedure reversed will greatly reduce her chances of being able to conceive easily. This is because the longer they wait the more damage will have been caused to their fallopian tubes, which in turn reduces the chances of the operation actually being successful.

Is Tubal Reversal a better option than IVF? - In a number of studies that have been carried out over the years relating to this matter, many women have found that having tubal reversal surgery rather than under going IVF (in vitro fertilization) treatment has been more successful.

One such study from the Chapel Hill Reversal Center, where they keep excellent track of their patients, it was discovered that the women who had the tubal reversal done, instead of the IVF treatments, conceived more easily. In addition, the number of actual babies born after they became pregnant, was higher for the tubal reversal surgery than for those who underwent IVF. Even better is that the reversal surgery is done generally on an outpatient basis with only about an hour in surgery thus lowering the cost of this treatment versus the IVF treatment which can take many tries.

All these items above need to be considered when you are making your decision concerning tubal ligation reversal. Keep it all in mind when you are looking at pregnancy after tubal reversal as any of the factors, or indeed all of the factors, could have an impact on your ability to conceive and have the child you are dreaming of. Pada kasus cairan air mani tanpa sperma (azoospermia), mungkin akibat penyumbatan atau gangguan saluran sperma, kini bisa dilakukan pengambilan sperma dengan teknik operasi langsung pada saluran air mani atau testis. Tekniknya ada dua, MESA (Microsurgical Sperm Aspiration) dan TESE (Testicular Sperm Extraction). Pada MESA, sperma diambil dari tempat sperma dimatangkan dan disimpan (epididimis). Sedangkan pada TESE, sperma langsung diambil dari testis yang merupakan pabrik sperma. Setelah sperma diambil, dipilih yang paling baik. Selanjutnya, dilakukan langkah-langkah menurut prosedur ICSI. Teknik ini juga sudah diterapkan di RSAB Harapan Kita sejak 1996 dan telah berhasil melahirkan dua anak.

Seperti di negara lain, sejak 1992 Indonesia sudah melakukan simpan beku embrio. Perangsangan indung telur wanita pada prosedur bayi tabung memungkinkan terbentuknya banyak embrio. Tidak mungkin semua embrio ditransfer ke dalam rahim pada saat bersamaan. Embrio yang untuk sementara tidak digunakan dapat disimpan dengan cara kriopreservasi, yang selanjutnya disimpan dalam tabung berisi cairan nitrogen pada suhu 196oC di bawah nol derajat. Kapasitas tabung sekitar 100 embrio.

Simpan beku embrio ini menghemat biaya karena pasangan tidak perlu lagi mengulang proses pengerjaan dari awal lagi bila embrio berikutnya perlu ditanamkan kembali. Tidak seperti di Barat, embrio ataupun sperma yang tersimpan beku di Indonesia hanya diperuntukkan bagi pasutri yang bersangkutan.

Salah satu contoh keberhasilan teknik penyimpanan embrio bisa ditemukan di Belgia. Baru-baru ini lahir seorang bayi laki-laki sehat hasil penanaman embrio yang sudah dibekukan selama 7,5 tahun dari pasangan lain (anonim). Bayi yang dibantu kelahirannya oleh dr. Michael Vermesh ini beratnya 4 kg. Daya tahan embrio yang dibekukan bisa puluhan tahun dan tetap bisa menjadi bayi sehat.

Teknologi reproduksi in vitro ternyata sangat membantu pasangan yang mengalami gangguan reproduksi. Mengupayakan pasutri agar bisa mempunyai anak sungguh merupakan perbuatan mulia dan membahagiakan, sekalipun pembuahannya dilakukan di laboratorium. Seperti halnya Louise Brown, mungkin banyak anak yang dilahirkan melalui teknik ini ikut bersyukur bahwa kedua orang tuanya mengikuti program itu.

Some people choose the nearest clinic to their home. This understandable as throughout the fertility treatment you will need to go back and forth, sometimes several times a day. If the ideal clinic for you, after all your research has been completed is a clinic 200 miles away, investigate the possibility of staying with friends, or a local hotel for a few weeks.

Hearsay and recommendation
Choosing a clinic solely on the fact that you heard a doctor there was ‘ good’ or you know someone who went there and has had success is also wrong. Sure, it provides a good place to start but again your research must go beyond this to identify what the clinic can do for you. Every couple’s circumstances is different and what works for one couple, may not work for another. Bear this in mind when researching the types of treatments available at each clinic.

Referral from Doctor or Gynaecologist
After completing your initial investigative tests, your doctor will undoubtedly have a conversation with you about what happens next. So far you have invested all your trust in your doctor and naturally respect his/her choice of clinic. Often, a doctors referral will be based on hearsay or anecdotal information. Certainly research your doctors recommendation but if the clinic does not suit you and your partner, do not feel pressured into it.

Cost
It is our nature to seek out a good deal. When it comes to fertility treatment there is no such thing as a good deal. The cost of treatment bears no relation to the clinics’ success rate. What does pay off is the time you invest in researching and asking the right questions.

Repeat treatment
You may already have completed one cycle of treatment at a chosen clinic and wish to try again. Do not feel under pressure to use the same clinic for your next attempt. If it has not worked the first time, there may be a valid reason for it.

The growing popularity of reproductive treatments has made patients much more aware of options like in vitro fertilization. The internet and other media streams allow potential candidates to do extensive research on infertility treatments even before they visit their doctor. While this new breed of educated patient is considered a good thing, it is important to remember that only a doctor can recommend the right infertility treatment.

Because in vitro approaches receive so much publicity, some couples are disappointed when they find out they don’t qualify. To clarify who is eligible, here are some of the common conditions treated with IVF.

-Endometriosis is a condition where additional tissue forms outside of the uterus, sometimes causing interference with other organs. Depending on the severity, this condition can prevent fertilization. By using IVF tactics to fertilize the egg outside of the body, doctors can sometimes bypass the negative affects of endometriosis.

-Low sperm counts in males decrease the chances of fertilization simply through a numbers game. The higher the sperm count, the more likely that a healthy sperm will reach the egg. IVF is used in these cases to join the egg and sperm together manually to guarantee a perfect union.

-Fallopian tube or uterus issues are another qualification factor for in vitro fertilization. When these components are damaged or not working properly, it is nearly impossible for a woman to conceive. While any form of fertility treatment involving these cases are not a sure bet, sometimes it is the only option for couples who are seeking pregnancy.

-Unexplained instances of infertility can also be treated with IVF. In these cases, doctors perform a variety of tests to determine the cause of the problem. If the results of the testing come back negative, then the case is considered to be unexplained. Typically, this rare scenario is treated with less invasive options such as fertility drugs or hormone therapy. However, if these solutions don’t work, then IVF becomes another possibility for the couple.

The preceding conditions are the most common qualifying factors for use of in vitro fertilization. All of them carry a similar trait in that the process of fertilization within the body is impaired in some way. Since IVF fertilizes a woman’s egg with the male’s sperm outside of the body, it can significantly improve chances of a successful pregnancy.

Forty may be the new thirty and fifty may be the new forty but nobody told our reproductive systems. And never has society provided more assistance in convincing us of our new younger status, via the mega industries of cosmetic enhancement and reproductive assistance.

Botox is to the face what IVF is to the ovaries – they both involve needles, both hurt like hell but one has a far greater success rate. No prizes for guessing which one. It’s far easier in your forties to look like Nicollette Sheridan than to reproduce like Cherie Blair. And the success rates with IVF, the most assisted of assisted reproductive techniques, is negligibly more than natural rates.

For instance at 45, there is a one percent chance of getting pregnant at all and then at least a fifty percent chance of miscarrying. The chances of IVF success between 40 and 45 is averaged out at ten percent but really starts at this and diminishes dramatically each year which is why many clinics will not perform a cycle for women over 42 using their own eggs.

They say it’s due to not wanting to take large sums of money in return for little or no hope but possibly they don’t want to deflate their own success rates as it isn’t good for business and this is understandable - they need business in order to improve their services.

By the time I was ready to embark on fresh rounds of IVF at the age of 41, having succeeded at 38*, my potential success rate had practically halved and the potential miscarriage rate had risen by fifty percent – hardly encouraging. Yet had I not been lucky enough to conceive a child already, no doubt I’d be in California now, having mortgaged my house and busy organizing an egg donor.

We can turn back the clock in so many ways but our eggs remain the same. I can only assume that in future more women will freeze their eggs when young, having learnt from a generation of women who found themselves, via modern circumstance, able to rid themselves of frown lines but unable to conceive a child.

Dulu hal ini mungkin akan terjadi atau malah sebaliknya tapi sekarang sebuah tehnologi baru dalam bayi tabung (in vitro fertilization) akan memberikan anda kesempatan dalam memilih jenis kelamin anak anda kelak. Pemilihan gender ini sekarang sedang hangat-hangatnya diperdebatkan dari segi etnis, moral maupun kelegalannya.

Pemilihan gender adalah sebuah metode yang digunakan untuk memiliki anak sesuai dengan jenis kelamin yang dikehendaki. Setiap sel telur memiliki satu kromosom X sementara sel sperma mengandung kromosom Y dan X. Ketika sel sperma dengan dominasi kromosom X membuahi sel telur maka akan melahirkan bayi perempuan dan sebaliknya, jika kromosom Y lebih dominan maka akan melahirkan bayi laki-laki. Maka peluang untuk melahirkan bayi laki-laki maupun perempuan adalah 50:50.
Tehnik pemilihan gender ini sudah ada sejak zaman Mesir Kuno. Dahulu, pemilihan gender dipengaruhi oleh posisi dalam hubungan seks, ketepatan waktu atau makanan khusus yang diberikan selama masa kehamilan. Tapi metode ini tidak didasari oleh penelitian ilmiah yang kuat dan sama sekali tidak meningkatkan peluang untuk memilih jenis kelamin bayi yang akan lahir kelak.

Alasan-alasan umum melakukan metode pemilihan gender:

Penyakit Keturunan
Beberapa penyakit keturunan berhubungan dengan jenis kelamin, seperti anak laki-laki yang rentan terhadap hemophilia dan pelemahan otot, maka para orang tua biasanya lebih memilih bayi perempuan.

Keseimbangan Keluarga
Pasangan yang sudah memiliki anak biasanya ingin memiliki anak lagi dengan jenis kelamin yang berbeda yaitu untuk 'menyeimbangkan' keluarganya.

Kematian Anak
Beberapa orang tua kehilangan anaknya di usia yang masih sangat muda. Metode pemilihan gender ini akan memberikan kesempatan pada orang tua untuk memiliki anak dengan jenis kelamin yang sama.

Alasan Lain
Adat istiadat, ekonomi keluarga dan alasan sosial lainnya.

Metode Pemilihan Gender:

Metode Gradient
merupakan salah satu metode yang paling sederhana dalam teknologi pemilihan gender. Sel sperma yang sudah ditampung kemudian diletakkan dalam mesin pemutar yang disebut Centrifuge. Dengan melalui proses pemutaran, sel sperma akan memisahkan kromosom Y dari kromosom X, yang lebih berat karena adanya materi genetik lainnya. Sel sperma yang dikehendaki kemudian akan digunakan dalam proses bayi tabung (in vitro fertlization) selanjutnya.
Metode gradient ini memiliki tingkat keberhasilan yang rendah tapi juga hanya memerlukan biaya yang lebih sedikit dari metode pemilihan gender lainnya.
Biaya: $600 per Cycle of IUI

Flow Cytometry
merupakan tehnik pemisahan sel sperma yang menggunakan fluorescent dye (substansi yang berguna untuk merubah warna) untuk menandai sperma yang mengandung kromosom X. Substansi ini menempel cepat pada materi genetik yang mengandung sperma. Karena sel sperm yang mengandung kromosom X lebih banyak memiliki materi genetik maka fluorescent dye ini akan menempel lebih banyak pada sel sperma ini dibandingkan dengan sel sperma yang mengandung kromosom Y. Kemudian mesin laser digunakan untuk memisahkan dua macam sel sperma tersebut yang lalu akan digunakan untuk proses bayi tabung.
Tingkat keberhasilan dari tehnik Flow Cytometry ini cukup tinggi, yaitu 60-70%.
Biaya: - ( terbatas untuk penelitian FDA)

Preimplantation Genetic Diagnosis (PGD)
PGD adalah metode yang terbaik dalam hal pemilihan gender. Metode ini merupakan prosedur yang rumit yang melibatkan penciptaan embrio yang kemudian dianalisis struktur sel dan DNA-nya. Embrio yang diciptakan dalam laboratorium dan kemudian membelah diri, lalu setelah 3-4 hari, satu sel dari setiap embrio yang membelah akan dipindahkan dan dianalisis DNA dan materi genetika-nya. Setelah jenis kelamin embrio tersebut diketahui, maka hanya embrio yang berjenis kelamin yang diinginkanlah yang kemudian akan diimplantasikan kedalam uterus si pasien melalui prosedur IVF.
Tingkat keberhasilan PGD nyaris sempurna, yaitu sekitar 99%.
Biaya: $2,000 - $4,000 (exclude IVF procedures cost)

Legal atau Ilegal?
Sekarang sedang hangat-hangatnya diperdebatkan di seluruh dunia. Ini dikarenakan metode pemilihan gender ini dipandang melawan moral dan kode etik oleh banyak orang. Beberapa pendapat mengatakan bahwa pemilihan gender ini membuat diskriminasi seksual dan stereotips. Banyak pula yang mengkhawatirkan bahwa metode pemilihan gender ini akan membawa efek negatif pada perbandingan pertumbuhan antara wanita dan pria di beberapa negara serta akan menimbulkan keinginan lain untuk memilih karakteristik anak-anak kita kelak, seperti: warna rambut, warna mata dan level intelegensi-nya. Karena alasan-alasan inilah beberapa negara melarang penggunaan metode pemilihan gender ini yang tidak disertai alasan medis yang kuat.
Metode pemilihan gender tersedia di hampir semua klinik fertilitas di Amerika, tapi tehnik ini hanya dapat digunakan untuk mereka yang memiliki masalah genetika. Pemilihan gender dengan tidak didasari alasan medis tidak diperkenankan di Inggris dan Kanada. Konferensi Genetika Manusia minggu ini menjelaskan bahwa semua embrio yang digunakan dalam prosedur In vitro fertilization (IVF) harus ditinjau ulang sebelum dilakukan implantasi untuk pemeriksaan kenormalan genetik yang terdapat didalam embrio tersebut.Dengan cara memonitor perkembangan embrio akan meningkatkan kemungkinan keberhasilan mendapatkan bayi yang sehat untuk wanita diatas 35 tahun yang menggunakan metode IVF.
Penelitian yang dilakukan terhadap 709 pasangan menunjukan bahwa 81% pasangan yang melakukan diagnosis embrio sebelum proses implantasi akan melahirkan bayi yang sehat sedangkan 19% sisanya kemungkinan akan melahirkan bayi abnormal atau bahkan tidak dapat memiliki bayi sama sekali. 40-70% embrio memiliki ketidaknormalan kromosom, maka dengan cara memilih embrio yang normal dan sehat yang lalu akan dipindahkan, kemungkinan bayi yang lahir adalah bayi yang sehat dan normal.
Diagnosis kromosom pada embrio pertama kali diperkenalkan pada tahun 1990 dan di negara Inggris Raya, penggunaannya masih terbatas, yakni hanya ditujukan bagi pasien pengidap fibrosis cystic (pembengkakan sistem urinal), penyakit Hunington atau bakteri tumor yang merupakan cikal bakal dari kanker usus. Setelah 5 hari, blastosis akan tiba di saluran uterus dan memulai proses implantasi. Pemindahan blastosis yang berumur 5 hari lebih bersifat alamiah dalam prosesnya jika dilihat dari padangan psikologi.

Ini karena selama proses ZIFT, sel telur anda akan dilihat berhasil atau tidaknya pembuahan yang dilakukan oleh si sel sperma dalam laboratorium. Lain lagi dengan tehnik Gamete Intrafallopian Transfer (GIFT) yang hanya meleburkan sel sperma dan sel telur lalu diletakkan langsung dalam tuba fallopi sebelum sel telur dan sperma tersebut menjadi sebuah zigot. Dr. Sudraji S, Pionir Bayi Tabung, Meninggal

Guru besar emeritus bidang obstetri ginekologi Fakultas Kedokteran Universitas Indonesia, Sudraji Sumapraja (72), meninggal, Selasa (14/8) pukul 11.30 di Rumah Sakit Cipto Mangunkusumo, Jakarta. Ahli infertilitas yang dikenal sebagai pelopor program bayi tabung di Indonesia itu meninggal akibat penyakit jantung dan diabetes dengan komplikasi gangguan ginjal dan hati.

Almarhum meninggalkan seorang istri (Iris Ambarwati), tiga anak (Hendrawati, Kanadi, dan Ambar Gunadi), serta tujuh cucu. Jenazah dimakamkan di Pemakaman Giritama Tonjong, Parung, Bogor, pada hari Rabu (15/8) dan diberangkatkan dari rumah duka, Jalan Ikan Hias Nomor 38A, Batu Ampar, Condet, Jakarta Timur, pukul 08.00.

Menurut ipar Sudraji, guru besar pulmonologi Fakultas Kedokteran Universitas Indonesia (FKUI) Hadiarto Mangunnegoro, 3 Agustus lalu almarhum sempat masuk Pusat Jantung Nasional Harapan Kita dan pulang pada 8 Agustus. Malamnya ia jatuh di kamar mandi dan sesak napas. Keesokan hari Sudraji dirawat di RSCM sampai akhir hayat.

Selasa siang jenazah disemayamkan di FKUI dan dilepas oleh Dekan FKUI Menaldi Rasmin beserta sivitas akademika UI.

Menurut Menaldi, Sudraji merupakan dokter yang patut diteladani dan sosok guru sejati. "Beliau bersikap sebagai teman bagi siapa pun, pekerja keras yang selalu tekun, rajin, sabar, santun, dan menuntun," kata Menaldi.

Sudraji menjadi Ketua Tim Rencana Strategis FKUI sejak 2000 sampai akhir hayat.

Saat menjadi Wakil Direktur Medis Kebidanan Rumah Sakit Anak dan Bersalin Harapan Kita, Sudraji mendirikan Program Melati (Melahirkan Anak Tabung Indonesia). Pada 2 Mei 1988 lahir bayi laki-laki hasil pembuahan tabung pertama di Indonesia.

Sudraji juga aktif di Yayasan Kesehatan Perempuan, Perkumpulan Kontrasepsi Mantap Indonesia, Yayasan Bina Pustaka Sarwono Prawirohardjo, dan sejumlah organisasi lain. (ATK)

Egg donation allows couples who want to have children but are finding it difficult to conceive. Typically, a woman's ovaries do not produce enough healthy eggs to become pregnant, she uses donated eggs. Healthy young women in their 20s to donate their eggs to help a couple to conceive or substitute through IVF.

What are the qualities of the egg donor?

Program standards vary, but most of the criteria have been set for legal reasons and to increase the chance of pregnancy. Egg donors must be between 21 years and 35, although most programs do not accept women over 30 years of age. You must be 21 to legally sign a contract and younger than 35 because older women tend to respond more poor to medicines that young women in their 20s.

To be eligible, women must be healthy, have no STD or disease, be non-smokers, have little or no genetic diseases in the family, had no psychological problems, these have no piercing, tattoos or body, we must continue or already obtained a university degree, and must be physically and emotionally able to donate. Many programs or ads also look ethnicity for certain circles, high IQ or standardized test scores, athletic talent, musical talents, and for "attractive" seeks donors.

Most of the criteria laid down is to ensure that both the young woman and her eggs are healthy, which will result in a healthy pregnancy and child. Couples tend to look for young women who look like them or have similar characteristics, which helps the couple feel that the child is usually act like them, as if it was genetically theirs.

Many programs offer more monetary compensation if you have already given birth or donated eggs successfully before. What does the egg donation process involve?

To apply for a program for egg donation, you must fill out the questionnaire, a long (averaging 30 pages), which requires that general information about yourself and details about you and your family's medical history . You will also be asked for photos of yourself, in most cases, from birth to the present time. Your request will be reviewed. If you are considered acceptable, you will be accepted into the program donated egg. Some programs at this stage in their position you egg donation database until you are selected by a couple. In vitro fertilisation (IVF) adalah sebuah proses untuk menghasilkan bayi di luar tubuh manusia. Sel Telur dari wanita dan sperma dari pria ditempatkan bersama-sama dalam piring plastik untuk menghasilkan terjadinya pembuahan. Seteleah terjadi pembuahan, telur yang sudah berubah menjadi embrio akan ditanamkan kembali ke belakang uterus wanita untuk dapat diproses secara alami oleh tubuh agar dapat terjadi kehamilan.

Prosedur Bayi tabung (IVF)
IVF bukanlah suatu prosedur yang mudah dan sederhana, tapi merupakan beberapa langkah prosedur yang dilakukan dalam beberapa minggu, seperti dijelaskan dibawah ini:

Menstimulasi Ovarium
Beberapa macam hormon akan disuntikan untuk menstimulasi ovarium untuk menghasilkan telur lebih dari pada masa subur umumnya. Perkembangan sel telur ini akan dipantau dengan cara tes darah dan dengan ultrasound. Cara ini digunakan untuk meyakinkan bahwa telur-telur yang dihasilkan pada saat yang tepat

Pengumpulan Sel Telur
Ultrasound dimasukkan dalam rahim wanita dan jarum yang sangat halus akan melacak perkembangan telur dalam rahim tersebut. Monitor pada Ultrasound akan menunjukan dimana follicle berada, kemudian jarum halus tersebut akan menindik follicle yang mengandung sel telur dan mengekstraksinya.

Fertilisasi dan Pemindahan Embrio
Dua jam setelah sel telur dikumpulkan, si pria akan mengeluarkan sperma, biasanya dilakukan lewat masturbasi. Dalam proses standar IVF, sel telur akan dicampurkan dengan sperma dalam satu wadah. Sedangkan dalam proses ICSI, satu sel sperma akan langsung diinjeksikan kedalam lapisan luar setiap sel telur yang ada.
Jika sel telur sudah dibuahi oleh sperma, akan menghasilkan sebuah zigot atau embrio muda. Zigot tersebut akan disimpan dalam inkubator selama 1-2 hari sampai membelah menjadi 2 atau 4 sel. Setelah proses pembuahan, 2 atau 3 embrio akan dipindahkan ke dalam uterus mengunakan catheter. Proses ini disebut pemindahan embrio dan cukup menimbulkan rasa sakit serta tidak memerlukan anastetik.
Sedangkan untuk proses Gamete Intrafallopian Transfer (GIFT), sel telur dan sperma diletakkan langsung dalam tuba fallopi dan membiarkannya melakukan proses yang alami. Prosedur ini menggunakan laparoskop dan anastetik umum, namun dewasa ini cara GIFT sudah jarang digunakan.

Male Infertility Treatments

Some causes of male infertility are sometimes correctable. A varicocele may be surgically repaired to improve fertility. Treatment with antibiotics of a chronic infection can enable a previously infertile man to become fertile. In some situations where substance abuse is a contributing factor, it may be essential for the male to abstain entirely from alcohol and/or other drugs and to join self-help groups in order to do so. Re-evaluation of medications prescribed to treat a chronic illness may produce positive results. A careful study of the man's exposure to occupational hazards such as radiation, lead, or dangerous pesticides may indicate a possible solution through change in employment.

In other cases, administration of various hormones can increase a borderline sperm count or suppress sperm antibodies enough to make conception possible. These hormones include testosterone, thyroid hormone and cortisone. In some situations clomiphene citrate (Clomid) or human menopausal gonadotropins (Pergonal), medications that are used to induce ovulation in infertile women, may also be given to a man whose pituitary deficiency is the cause of his inability to father an offspring. In vitro fertilization, originally used more for female infertility, is being used increasingly for the treatment of male infertility.

What is being described as a revolution in treating infertile men originated in Belgium in 1993, when researchers produced several successful pregnancies by the direct injection of a single sperm cell into a human egg in a Petri dish. The important discovery was that men who had no viable sperm in their semen often had at least a small number in their testes. The problem was that getting the sperm out of the testicles required a very expensive operation and an extended hospital stay.

In 1995, American researchers found a much easier and cheaper way to extract the sperm: by aspirating them through a thin needle in a procedure that can be done in the doctor's office. Even though the needle aspiration is not very expensive, it has to be combined with in vitro fertilization and the direct injection of sperm into eggs. The combined procedures, known as intracytoplasmic single sperm injection (ICSI), can cost as much as $15,000, an amount not likely to be covered by insurance.

The extraordinary advantage of this new development is that it has reduced fertilization to getting the sperm's genes into the egg. It doesn't matter whether the sperm can swim vigorously or even if it can penetrate the egg's outer layer. All that matters is that it is alive. Dr. Richard J. Sherins, director of the male infertility program at the Genetics and IVF Institute in Fairfax, Virginia and the developer of the aspiration technique, believes that it should be of the greatest use to the approximately ten million American men who have had vasectomies. This is encouraging news because while the vas may be surgically repaired, this does not always result in the resumption of fertility. And according to the "New York Times" (6/19/95), even though the method is expensive, it has resulted in a diminishing market for sperm donors both in this country and Europe.

Keajaiban itu bernama Carine, seorang bocah berusia sebelas bulan yang lahir di Kanada. Bocah perempuan ini lahir dari metode pembuahan di laboratorium, ditumbuhkan dari sel telur yang dibekukan lantas dibuahi di tabung laboratorium.

Carine lahir dari seorang ibu yang mengidap sindrom kista di indung telur. Penyakit ini biasanya menyebabkan penderitanya mandul. Di Inggris, satu dari lima perempuan menderita penyakit ini, termasuk Victoria Beckham, istri bintang sepakbola David Beckham.

Ibunda Carine adalah satu dari 20 sukarelawan yang mencoba metode pembuahan baru yang dihasilkan peneliti dari Pusat Reproduksi McGill di Montreal, Kanada. Sel telurnya dimatangkan di laboratorium, dibekukan, lantas dicairkan, lalu dibuahi dalam tabung.

Pematangan sel telur di laboratorium sudah pernah dilakukan sebelumnya dan melahirkan bayi yang sehat. Pembekuan sel telur pun terbilang sebagai prosedur yang sudah mapan. Namun Carine adalah kasus pertama di dunia, terlahir dari penggabungan seluruh prosedur itu.

Kelahiran Carine baru diungkapkan ke publik dalam pertemuan tahunan Masyarakat Reproduksi dan Embriologi Manusia Eropa (ESHRE) di Lyon, Prancis, pada senin lalu. Doktor Hananel Holzer dari McGill mengatakan keberhasilan itu memberi harapan bagi para perempuan mandul akibat kista.

Holzer mengatakan selain kelahiran pertama tersebut, masih ada tiga perempuan lain yang sedang hamil dengan cara yang sama dan siap melahirkan bayinya dalam waktu dekat.

Ibunda Carine dan para sukarelawan lain rata-rata berusia 30an tahun dan sama-sama mengalami penyakit kista di indung telur (polycystic). Para peneliti di McGill mengumpulkan 295 oosit (sel telur yang belum matang) dari indung telur mereka dan mematangkannya dengan bantuan hormon di laboratorium selama lebih dari 48 jam sebelum dibekukan.

Sebanyak 68 persen sel telur berhasil dimatangkan. Sel-sel telur itu lantas dibekukan tak lebih dari sebulan sebelum dicairkan kembali. Ternyata 74 persen (64 embrio) berhasil bertahan hidup setelah dicairkan dan dibuahi dengan teknik injeksi sperma.

Embrio-embrio itu lantas ditransfer ke kandungan para sukarelawan. Para peneliti mentrasfer masing-masing tiga sel telur kepada tiap-tiap pasien. Pada empat pasien, proses ini berhasil dan berujung pada kehamilan, bahkan salah satunya berhasil melahirkan Carine.

Holzer mengatakan, teknik baru itu akan bermanfaat bagi perempuan yang mengidap kanker, seperti kanker payudara. Perempuan seperti ini umumnya akan memilih cara pembuahan in vitro fertilization (IVF) alias bayi tabung. Tindakan kemoterapi yang mereka jalani biasanya akan membuat mereka mandul.

Namun, dalam proses bayi tabung, mereka harus mendapat terapi hormon yang berguna untuk merangsang produksi sel telur di dalam ovarium. Terapi semacam ini justru memperburuk penyakit kanker yang mereka derita.

Stimulasi ovarium secara tradisional itu juga memakan waktu, antara dua hingga tiga pekan. Mereka yang menderita sindrom stimulasi berlebihan (ovarian hyperstimulation syndrome), takkan punya waktu untuk menunggu metode tradisional. Sindrom ini dapat berakhir pada kematian.

Itulah sebabnya, menurut Holzer, teknik itu akan membantu mereka menyimpan dan membekukan beberapa sel telur sebelum terlambat. Dan pemberian hormon pun cukup dilakukan di laboratorium, bersamaan dengan proses pematangan sel telur.

Sebelum penemuan itu diumumkan, dunia kedokteran belum mengetahui bahwa sel telur yang belum matang bisa dikumpulkan dari indung telur yang belum terstimulasi. Apalagi bahwa sel telur itu bisa dimatangkan lalu dibekukan, lantas dicairkan, sebelum dibuahi dan dipindahkan ke kandungan seorang perempuan yang kemudian hamil karenanya.

Tapi Holzer memperingatkan, penelitian itu masih pada tahap awal dan belum diujicoba pada pasien kanker. "Untuk metode penyimpanan fertilitas, ini masih tahap awal dan eksperimental," kata Holzer. "Kami perlu menginformasikan kepada pasien tentang tahap awal ini, supaya mereka tak menyimpan harapan yang keliru."

Namun teknik ini diharapkan bisa menggantikan praktek yang selama ini ditawarkan kepada pasien kanker, yakni para dokter secara sederhana akan membekukan keseluruhan jaringan indung telur yang bisa diimplan kemudian. Cara ini menimbulkan kekhawatiran bahwa jaringan yang akan diimplan kembali itu tak bebas dari sel kanker.

Perempuan sehat pun bisa memanfaatkan metode baru ini untuk menyimpan kesuburan mereka pada usia pertengahan tanpa perlu menginjeksi hormon. "Perempuan pada usia pertengahan 30an tahun tanpa pasangan dapat membekukan sel telur mereka dengan cara ini," katanya Holzer.

Joep Geraedts, Ketua ESHRE, mengatakan bila nanti metode itu terbukti pada pasien penderita kanker maka itu juga mungkin bagi semua perempuan penderita kista atau reproduksi buatan. "Karena mereka tidak perlu lagi diganggu dengan hormon," kata Geraedts.

Di samping itu, metode itu juga menghemat biaya. Pasalnya perawatan dengan obat-obatan hormon terlalu mahal.

Doktor Allan Pacey, ahli andrologi dari Universitas Sheffield di Inggris, mengatakan keberhasilan para peneliti dari Kanada adalah langkah maju yang signifikan. "Bandingkan dengan lelaki yang dengan mudah menyimpan spermanya sebelum perawatan kanker, perempuan dulu hanya memiliki sedikit pilihan dan kesannya sungguh tidak adil," katanya.

Menurut Pacey, kini yang mesti dilakukan lebih lanjut adalah memastikan bahwa teknik itu aman dan bayi yang lahir pun sehat. "Bila ini sudah dicapai, maka metode ini menjadi amat penting," ucapnya.

Professor Robin Lovell-Badge dari Institut Nasional Riset Kedokteran Dewan Riset Kedokteran mengatakan seluruh langkah dalam metode itu pernah dilakukan sebelumnya. "Namun ini kali pertama semuanya dilakukan bersamaan dan sukses."

Adapun Doktor Laurence Shaw, juru bicara Masyarakat Fertilitas Inggris mengatakan, kehamilan dan kelahiran itu adalah langkah yang mengagumkan. Namun dia mengingatkan bahwa angka kehamilan dari para sukarelawan masih terbilang rendah dan dibutuhkan sel telur dalam jumlah yang besar.

Measuring hormone levels not linked to blood or urine may also yield information about infertility. For example, certain conditions which are associated with abnormally high male hormones such as testosterone or cortisonelike hormones can cause infertility. Also follicle-stimulating hormone (FSH) and luteinizing hormone (LH) are two messenger hormones that play a vital role in the delicate process of ovulation. If these conditions are slightly reduced or introducing large amounts do not fluctuate or appropriate in the month, can lead to infertility. The hormone prolactin (which plays an important role in the production of milk) may be abnormally high, and be the cause of infertility. The treatment of this high level of hormone drugs Bromocryptine in many cases, the cure infertility.

A hysterosalpingogram is a test used for the study of the uterus and fallopian tubes. It can be done in a hospital or in the office of a radiologist. A speculum is inserted into the vagina and the cervix is apprehended with a tenaculum. A breakthrough dye injection is then attached to the cervix, the stain is slowly injected into the uterus and X-rays are taken. Most women feel the injection of dye to be about as uncomfortable as moderate cramps. Radiographs showed domestic routes of the uterus and the tubes of dye fills. This test also gives valuable information to a DES exposed to a woman before pregnancy. The degree of abnormality on the shape of the uterus is to predict the chances of premature labor during pregnancy and will help determine how closely such as the woman must be followed during pregnancy.

A trial postcoital (PC test) is a pain, simple test, which often can yield important information in the evaluation of infertile couples. This test is performed at the time of ovulation. You must come to the doctor in a number of hours after intercourse. A review is made speculum and a small sample of cervical mucus and vaginal secretions were taken and examined miscroscopically. This review will show whether cervical mucus is normal and if the sperm are active and alive. If sperm are alive and active, it is presumptive evidence that sperm antibodies are not a problem.

Laparoscsopy is often the last step of an infertility workup. This is done in a hospital, mostly with general anesthesia, although local anesthetic can be used. A small incision is made just below the navel and a long needle is inserted into the abdominal cavity. The abdominal cavity is filled with carbon dioxide. Called laparoscope, a long, narrow, lighted tube is inserted into the abdominal cavity to allow visualization of the pelvic organs. Dye is injected into the uterus. The doctor can look through the laparaoscope and see if the dye spreads the ends of the fallopian tubes, thus determining whether the tubes are open or blocked. In addition, laparoscopy can diagnose endometriosis, pelvic adhesions, and previous pelvic infections.

The miracle of life with the right infertility treatment can be a full success and allow couples who would not otherwise be able to have a child. People who cannot have children but desperately want children are very passionate about finding the right treatment, procedure and people the help them achieve what they consider to be a dream. It is not to be taken lightly in any means and a baby is a precious little life that needs to be protected once the process starts.

Memfatwakan :

1. Bayi tabung dengan sperma dan ovum dari pasangan suami isteri yang sah hukumnya mubah (boleh), sebab hak ini termasuk ikhtiar berdasarkan kaidah-kaidah agama.

2. Bayi tabung dari pasangan suami-isteri dengan titipan rahim isteri yang lain (misalnya dari isteri kedua dititipkan pada isteri pertama) hukumnya haram berdasarkan kaidah Sadd az-zari'ah, sebab hal ini akan menimbulkan masalah yang rumit dalam kaitannya dengan masalah warisan (khususnya antara anak yang dilahirkan dengan ibu yang mempunyai ovum dan ibu yang mengandung kemudian melahirkannya, dan sebaliknya).

Kisah Pasutri Batam yang Sukses Ikut Bayi Tabung di Malaysia

Beberapa pasangan suami-istri (pasutri) di Batam yang ingin punya anak melalui program bayi tabung atau teknik in vitro fertilization lebih suka pergi ke Mahkota Medical Centre di Melaka, Malaysia. Mereka pun berhasil dan sebagian besar mendapatkan bayi kembar. Mengapa mereka memilih Malaysia?

Hari sabtu, sekitar pukul 10.30, RS Harapan Bunda (RSHB) Batam punya gawe. Bertempat di lantai lima rumah sakit itu, beberapa pasutri datang sambil menggendong bayinya. Mereka adalah peserta program bayi tabung yang sukses ditangani Mahkota Medical Centre (MMC), Melaka, Malaysia.

Beberapa pasutri dengan bayi-bayinya itu langsung disambut dokter dan petugas medis dari MMC yang hari itu berada di RSHB untuk mempresentasikan program bayi tabung.

RSHB dan MMC memang akan menjalin kerja sama untuk program bayi tabung dengan teknik in vitro fertilization. Para pasutri yang diundang hari itu adalah warga Batam yang lebih dahulu berhasil menjalani program bayi tabung. Mereka sengaja diundang ke rumah sakit yang terletak di kawasan bisnis Nagoya, Batam, itu untuk berbagi cerita seputar program yang dijalani.

Di antara mereka adalah pasangan Dodi, 37, dan Magdalena, 36. Pasutri yang tinggal di kawasan Eden Park, Batam Centre, itu datang dengan menggendong dua balita perempuan kembar. "Ya, ini kembar," kata Lena –panggilan akrab Magdalena– dengan senyum bahagia kepada Batam Pos (Grup Jawa Pos).

Dia lantas menyebut nama dua putri kembarnya. Yang lebih tua Andrea Safina Kahla digendong Lena, dan adiknya Audrey Zafira Khansa digendong ayahnya. Kahla tampak lebih agresif. Sedangkan adiknya, Khansa, lebih tenang.

Lena yang hari itu mengenakan baju merah dan berjilbab menceritakan, kedua anak perempuannya tersebut adalah hasil program bayi tabung yang diikuti di pusat bayi tabung MMC. ”Sebelumnya, kami berumah tangga selama enam tahun,” katanya.

Bayi kembar itu lahir pada 8 Oktober 2005. ’’Tanggal 8 nanti (Juli), usianya sembilan bulan,’’ ujar Lena.

Mengapa memilih mengikuti program bayi tabung hingga ke MMC di Melaka? ”Kami enam tahun tak dikaruniai momongan sejak menikah 1999. Padahal, menurut dokter, tidak ada yang salah pada saya dan suami saya,” tuturnya.

’’Selama enam tahun itu, kami sudah berobat ke sana kemari. Ikut program di Jakarta, berobat tradisional, berobat ke dokter kandungan, tapi enggak dapat-dapat,’’ katanya sambil menggoyang-goyang Kahla yang berada dalam gendongannya.

Dari bertanya ke beberapa teman, akhirnya Lena dan suaminya mendapatkan informasi tentang program bayi tabung di MMC Melaka, Malaysia. ’’Desember 2004, kami mencoba ke Melaka dan ikut program bayi tabung,’’ kisahnya.

Setelah melalui pemeriksaan kondisi, 10 Januari 2005, dokter di MMC menanam embrio di rahim Lena. Ketika itu Lena ditangani dr S. Selva, spesialis kandungan dan kebidanan di MMC. Upaya ini membuahkan hasil. Hanya dua pekan setelah penanaman embrio, Lena hamil.

”Dokter minta setelah sepuluh hari supaya tes. Saya cek dengan alat cek kehamilan biasa. Saya bersama suami melakukan tesnya. Ternyata, saya hamil, hasilnya positif. Rasanya senang sekali,” katanya sambil tersenyum. ”Kami waktu itu langsung sujud syukur sambil menangis haru,” lanjutnya.

Saat itu sebenarnya Lena berharap dikaruniai satu anak saja. Tapi, ternyata mereka diberi dua momongan sekaligus. Kahla dan Khansa pun lahir. ”Rumah pun semakin ramai,” ucapnya.

Ketika ditanya soal biaya, Lena menyebut angka sekitar 12 ribu ringgit Malaysia (sekitar Rp 30 juta, dengan kurs 1 ringgit = Rp 2.500). ”Itu belum termasuk biaya perjalanan PP dari Batam ke Melaka.

Lena mengatakan, biaya untuk bayi tabung di MMC bergantung proses. Semakin sulit prosesnya, semakin mahal biayanya. Paling mahal 15 ribu ringgit (sekitar Rp 37,5 juta).

Perjalanan ke Melaka bisa ditempuh dengan kapal feri dari Batam ke Johor Baru dulu. Ini butuh waktu sekitar 1 jam 45 menit. Dari Johor Baru ke Melaka naik bus, kurang lebih 4 jam. ”Bagi kami memang lebih praktis dan murah ke Melaka daripada ke Jakarta. Karena itu, jika kerja sama antara RSHB dan MMC terealisasi, bayi tabung bisa ditangani di sini (Batam),” katanya.

Pasutri asal Batam yang juga berhasil menjalani program bayi tabung atau in vitro fertilization di MMC adalah Mujianto, 38, dan Jenny, 36. Keluarga yang tinggal di kawasan Dutamas, Batam Centre ini sebenarnya tidak punya rencana memperoleh momongan melalui program bayi tabung. ”Saya tidak ada rencana. Tadinya cuma mengantar Mama berobat ke MMC. Tapi, saya juga ikut cek. Dari hasil USG, dokter kandungan merekomendasikan agar kami ikut program bayi tabung. Katanya ini satu-satunya jalan yang harus dilakukan (untuk dapat hamil),” ungkap wanita yang bekerja di Asuransi AIA ini.

Selama enam tahun pernikahannya dengan Mujianto yang menjabat sebagai home loan manager di BII, Jenny tidak pernah mempersoalkan belum hadirnya buah hati dalam keluarganya. Meski demikian, Jenny juga tidak habis-habisnya berusaha secara medis.

”Saya berusaha secara medis, tidak pernah ke dukun. Saya percaya pada Tuhan saja. Secara medis, mulai terapi hingga operasi. Waktu itu katanya ada penyumbatan di rahim saya. Saya bahkan berkali-kali ditiup,” ceritanya.

Rupanya, jodoh pasutri Mujianto dan Jenny ini ada di MMC. Awal 2002, pasangan Jenny-Mujianto memulai program bayi tabung (IVF). Jenny-Mujianto merupakan pasangan pertama dari Batam yang mengikuti program bayi tabung di MMC. Hasilnya, sebulan sejak embrio ditanamkan, Jenny pun hamil.

”Saya bersama suami tes sendiri. Benar-benar heboh waktu tahu saya hamil. Ini anugerah terbesar, bisa melihat keajaiban yang diberikan Tuhan. Ini (anak) kan impian setiap orang,” ucap Jenny dengan raut wajah berbinar.

Sepuluh bulan kemudian, penantian Jenny dan Mujianto pun berakhir. Dua bayi laki-laki lahir pada 30 November 2002. Bayi kembar itu akhirnya diberi nama Jovan Theo Anthony dan Jonas Theo Anthony. Saat ini keduanya sudah berusia 3,5 tahun. ”Mereka kompak, mau apa-apa kompak. Tapi, sifatnya berbeda,” ungkap Jenny.

Selain usaha yang cukup panjang, keluarga yang mengikuti program bayi tabung dengan teknik in vitro fertilization ini harus merogoh kocek lebih dalam. Berapa biaya yang harus dikeluarkan? Vincent Wan, marketing director MMC membeberkan, untuk bayi tabung biayanya rata-rata 12 ribu ringgit Malaysia atau sekitar Rp 30 juta.

In-vitro fertilization or IVF is a technological advances in the medical field in response to the alarming rate of infertility in developed countries. The concept was launched by Patrick Steptoe and Robert Edwards, a physiologist at Cambridge in the late 1960's and a decade later, the first IVF baby was delivered in the UK.

IVF was invented mainly to help women who suffer from infertility due to blocked fallopian tubes or uterine sick, but over time, the procedure has been extended to cover cases such as endometriosis, cervical mucus hostile and a host of other infertility problems. It is even now applied in cases where the husband or male partner suffers from a low sperm count, but the results with them have not been particularly impressive.

The process of in vitro fertilization, for the most part, involves the use of a laparoscope, a telescope, which can be inserted into the abdomen of women under anesthetic conditions, to remove the eggs from the ovary in women of a period just before the time when it would naturally be released (Ovulé). Harvesting egg is then mixed with washed and diluted male sperm in a glass dish. If fertilization occurs between the egg and sperm, the resulting embryo is allowed to develop in the laboratory, usually for two to three days before the embryo is then implanted or reintegrated into the uterine cavity the woman with a kind of plastic tube.

Like any other man the procedure, the IVF has its own share of limitations and adverse effects. The success rate of the procedure and the risk of suffering an ectopic pregnancy, a pregnancy that implants and develops outside the uterus and can be found almost always broke with tube to the uterus, are two of these issues. Metode bayi tabung yang dipelopori sejumlah dokter Inggris ini untuk pertama kali berhasil menghadirkan bayi perempuan bernama Louise Brown pada tahun 1978. Sebelum ditemukannya teknik bayi tabung, untuk menolong pasutri tak subur digunakan teknik inseminasi buatan, yakni dengan cara penyemprotan sejumlah cairan semen suami ke dalam rahim dengan bantuan alat suntik. Dengan cara ini diharapkan sperma lebih mudah bertemu dengan sel telur. Sayang, tingkat keberhasilannya hanya 15%.

Pada teknik bayi tabung atau in vitro fertilization yang melahirkan Louis Brown, pertama-tama dilakukan perangsangan indung telur sang istri dengan hormon khusus untuk menumbuhkan lebih dari satu sel telur. Perangsangan berlangsung 5 - 6 minggu sampai sel telur dianggap cukup matang dan sudah saatnya diambil. Selanjutnya, folikel atau gelembung sel telur diambil tanpa operasi, melainkan dengan tuntunan alat ultrasonografi transvaginal (melalui vagina).
Sementara semua sel telur yang berhasil diangkat dieramkan dalam inkubator, air mani suami dikeluarkan dengan cara masturbasi, dibersihkan, kemudian diambil sekitar 50.000 - 100.000 sel sperma. Sperma itu ditebarkan di sekitar sel telur dalam sebuah wadah khusus di dalam laboratorium. Sel telur yang terbuahi normal, ditandai dengan adanya dua sel inti, segera membelah menjadi embrio. Sampai dengan hari ketiga, maksimal empat embrio yang sudah berkembang ditanamkan ke rahim istri. Dua minggu kemudian dilakukan pemeriksaan hormon Beta-HCG dan urine untuk meyakinkan bahwa kehamilan memang terjadi.

Sejak kelahiran Louise Brown, teknik bayi tabung atau In Vitro Fertilization (IVF) semakin populer saja di dunia. Di Indonesia, teknik bayi tabung (IVF) ini pertama kali diterapkan di Rumah Sakit Anak-Ibu (RSAB) Harapan Kita, Jakarta, pada 1987. Teknik bayi tabung yang kini disebut IVF konvensional itu berhasil melahirkan bayi tabung pertama, Nugroho Karyanto, pada 2 Mei 1988. Setelah itu lahir sekitar 300 "adik" Nugroho, di antaranya dua kelahiran kembar empat.

Sukses besar teknik bayi tabung (IVF) konvensional ternyata masih belum memuaskan dunia kedokteran, apalagi kalau mutu dan jumlah sperma yang hendak digunakan kurang. Maka dikembangkanlah teknik lain seperti PZD (Partial Zona Dessection) dan SUZI (Subzonal Sperm Intersection). Pada teknik PZD, sperma disemprotkan ke sel telur setelah dinding sel telur dibuat celah untuk mempermudah kontak sperma dengan sel telur. Sedangkan pada SUZI sperma disuntikkan langsung ke dalam sel telur. Namun, teknik pembuahan mikromanipulasi di luar tubuh ini pun masih dianggap kurang memuaskan hasilnya.

Sekitar lima tahun lalu Belgia membuat gebrakan lain pada teknik bayi tabung yang disebut ICSI (Intra Cytoplasmic Sperm Injection). Teknik canggih ini ternyata sangat tepat diterapkan pada kasus mutu dan jumlah sperma yang minim. Kalau pada IVF konvensional diperlukan 50.000 - 100.000 sperma untuk membuahi sel telur, pada ICSI hanya dibutuhkan satu sperma dengan kualitas nomor wahid. Melalui pipet khusus, sperma disuntikkan ke dalam satu sel telur yang juga dinilai bagus. Langkah selanjutnya mengikuti cara IVF konvensional. Pada teknik ini jumlah embrio yang ditanamkan cuma 1 - 3 embrio. Setelah embrio berhasil ditanamkan dalam rahim, si calon ibu tinggal di rumah sakit selama satu malam.

Di Indonesia, menurut dr. Subyanto DSOG dan dr. Muchsin Jaffar DSPK, tim unit infertilitas MELATI-RSAB Harapan Kita, ICSI sudah diterapkan sejak 1995 dan berhasil melahirkan anak yang pertama pada Mei 1996. Dengan teknik ini keberhasilan bayi tabung meningkat menjadi 30 - 40%, terutama pada pasangan usia subur.

Berdasarkan pengalaman, menurut dr. Muchsin, peluang terjadinya embrio pada teknologi bayi tabung sekitar 90%, di antaranya 30 - 40% berhasil hamil. Namun, dari jumlah itu, 20 - 25% mengalami keguguran. Sedangkan wanita usia 40-an yang berhasil melahirkan dengan teknik in vitro hanya 6%. Karena rendahnya tingkat keberhasilan dan mahalnya biaya yang harus dikeluarkan pasien, teknik ini tidak dianjurkan untuk wanita berusia 40-an.

Pasangan yang masuk program MELATI tidak harus mengikuti program IVF. Teknik ini hanya ditawarkan kalau setelah diusahakan dengan cara lain, tidak berhasil. Sebelum mengikuti program ini pun pasutri diminta mengikuti ceramah dan menerima penjelasan semua prosedurnya agar diikuti dengan mantap.

Biaya mengikuti program bayi tabung (IVF) ini memang tidak murah. Pada akhir 1980-an biayanya sekitar Rp 5 juta. Kini, berkisar antara Rp 13,5 juta - Rp 18 juta. Harga obat suntik perangsang indung telur saja sudah naik hampir empat kali lipat. Padahal, suntikan yang dibutuhkan selama dua minggu mencapai 45 ampul.

Selain RSAB Harapan Kita, Jakarta, teknik bayi tabung (IVF) juga sudah diterapkan di FKUI-RSUPN Cipto Mangunkusumo (Jakarta), Fakultas Kedokteran Universitas Airlangga (Surabaya), dan Fakultas Kedokteran Universitas Gadjah Mada dan RS Dr. Sardjito (Yogyakarta).
